What the Masseter Is and How Treatment Works

The masseter is your main chewing muscle, running from the cheekbone down to the angle of the jaw. You can feel it tighten under your fingers if you clench. In some people it is naturally bulky; in others it has enlarged through years of clenching, grinding or chewing habits, much as any muscle enlarges with repeated use.

Botulinum toxin reduces the muscle’s activity, and with reduced activity the muscle gradually reduces in size over the following weeks and months. That gradual reduction is what narrows the lower face, and it is also why nothing appears to happen for the first several weeks. Where clenching is the problem, the reduced activity itself provides the relief, and that arrives sooner.

Is Your Jaw Wide Because of Muscle, or Bone?

This is the assessment that determines whether the treatment is worth having, and it is the step most often skipped elsewhere. Lower face width has three quite different sources.

Consequently we ask you to clench during the examination and we palpate the area. If your width is skeletal, no amount of toxin will change it, and we will say so rather than sell you a course. Many patients have a combination, in which case treatment softens part of the width but not all of it — and knowing that in advance is what prevents disappointment three months later.

If You Grind Your Teeth, Start With Your Dentist

Botulinum toxin can reduce clenching force and many patients report genuine relief from jaw ache, morning tightness and tension-type headaches. Nevertheless, it is not the first step, and we would rather explain why than simply take the booking.

Grinding damages teeth, and a dental assessment establishes what damage has already occurred and whether a night guard or splint is appropriate. Splint therapy is the established first-line approach, it protects the teeth in a way toxin does not, and it is considerably cheaper. Additionally, grinding is often driven by stress, sleep disruption or, in some cases, obstructive sleep apnoea, and none of those is addressed by relaxing a muscle.

Therefore our recommended pathway is dental assessment first, splint where indicated, and botulinum toxin added if symptoms persist or if the muscle bulk itself is a concern. Used that way it works well as part of a plan rather than as a substitute for one.

Timelines: Slower Than You Expect

Patients who have had upper-face treatment expect a similar timeline here, and they are usually surprised. The two indications also differ from one another.

  • Clenching relief — usually noticeable within one to two weeks.
  • Visible slimming — begins around six weeks, with the fullest effect at roughly three months.
  • Duration — typically four to six months, longer than most facial areas.
  • Cumulative effect — two or three treatments across the first year usually produce more slimming than one, after which maintenance is less frequent.

Accordingly, we review at six to eight weeks rather than the two weeks used for the upper face, because at two weeks there is genuinely nothing to assess. And if you are planning around a wedding or event, start at least four to six months ahead. This is not a treatment that can be rushed.

Dosing

Masseter treatment uses substantially higher doses than upper-face areas, because the muscle is large and strong. Our typical range is per side, adjusted for muscle bulk, and asymmetry between sides is common and dosed accordingly. Injections are placed within the muscle at defined points, avoiding the areas where product could affect nearby muscles of expression.

Risks Specific to This Area

These differ from the upper face and deserve reading properly before you book.

  • Chewing fatigue — common in the first two to three weeks, particularly with tough or chewy foods. It settles as you adapt.
  • Paradoxical bulging — a recognised effect where other parts of the muscle compensate and bulge on clenching. It can look odd and generally resolves, but it is worth knowing about.
  • Smile asymmetry — uncommon, and caused by product reaching nearby muscles of expression. Placement is what prevents it, which is why this area needs experience.
  • A gaunt or hollow appearance — the masseter contributes support to the lower face. In slim faces, or with repeated over-treatment, reduction can read as hollow rather than sculpted.
  • Loss of definition with age — in older patients with existing laxity, reducing lower-face volume can make jowling more apparent rather than less.
  • Bruising and tenderness — usually minor and short-lived.

For the last two reasons in particular, we treat conservatively and build gradually rather than aiming for maximum reduction in one session. Reduction is easy to add and slow to reverse.

Who This Suits, and Who It Does Not

It suits patients with genuinely enlarged masseter muscles who want a softer lower-face outline, patients whose clenching persists despite a dental splint, and patients with jaw ache or tension headaches linked to clenching where a dental assessment has already been done.

It does not suit patients whose jaw width is skeletal, patients hoping to change facial fullness caused by fat, very slim faces where reduction risks a hollow look, or anyone with significant temporomandibular joint dysfunction that has not been properly assessed. In that last case we would want a dental or maxillofacial opinion first, since jaw joint problems and muscle bulk are different issues with different management.

Aftercare

Stay upright for four hours and avoid massaging or pressing the treated area for 24 hours. Skip the gym, sauna and steam room for a day. Expect some tenderness on chewing and consider softer foods for the first week or two if it bothers you — this is normal rather than a complication. Mild bruising at injection points is possible. Contact us if you develop marked asymmetry, difficulty opening your mouth, or anything that concerns you.

Who Should Not Have It

Treatment is avoided in pregnancy and breastfeeding, with infection at the injection site, with known allergy to any component including albumin, and in neuromuscular conditions such as myasthenia gravis. Significant untreated temporomandibular joint dysfunction should be assessed before any masseter treatment.

Will masseter Botox slim my jaw?

Only if your jaw width comes from enlarged muscle. Where the width is skeletal, or caused by facial fat, this treatment will not change it. We assess by asking you to clench and palpating the muscle, and we will tell you honestly which applies to you.

How long before I see slimming?

Is it painful?

Does it help TMJ pain?

It may help where the pain is driven by muscle overactivity and clenching. However, temporomandibular joint dysfunction is a distinct problem and should be properly assessed first, since joint pathology needs different management from muscle bulk.
