Why Forehead and Frown Lines Are Treated Together

This is the single most important thing on the page, and it explains most poor outcomes elsewhere. The frontalis, running across your forehead, is the only muscle that lifts your eyebrows. The glabellar muscles between your brows pull them down. The position of your brow at rest is the outcome of that tug-of-war.

Now consider what happens if only the forehead is treated. You have weakened the sole lifter while leaving the depressors at full strength, and the brow consequently sits lower. Patients describe this as heaviness, a hooded feeling, or eyes that look tired — and it is a predictable mechanical result, not bad luck.

Consequently, forehead treatment is planned alongside the glabella, so the depressors are softened as the lifter is. Conversely, treating the glabella alone is perfectly reasonable and often produces a slight brow lift as a bonus. So the two areas are not equally optional: forehead-only treatment is the combination we would generally decline.

Brow Position and the So-Called Brow Lift

A modest brow lift is achievable by treating the muscles that pull the brow down — the glabellar complex and the outer part of the muscle circling the eye — while leaving the lifter to work unopposed. The effect is real but measured in millimetres, typically one to three, and lasts as long as the treatment does.

It is not a surgical brow lift and will not correct significant hooding or excess upper eyelid skin. Where that is the concern, an oculoplastic or surgical opinion is the honest answer. We would rather refer than take payment for something that cannot deliver what you want.

Heavy or Hooded Brows Need Caution

Some patients arrive with brows already sitting low, sometimes unconsciously compensating by holding the forehead raised most of the time. Those horizontal forehead lines are, in effect, doing a job.

Treating such a forehead conventionally removes that compensation and the brow drops further, which makes the hooding worse rather than better. Therefore, in this pattern we either treat the depressors only, or use very conservative forehead dosing placed higher, or advise against forehead treatment altogether. Recognising this pattern before injecting is the difference between a good result and three months of regret, and it is something we specifically check.

Men and Women Are Dosed Differently

Two differences matter clinically. Men generally have stronger facial muscles and typically require higher doses for an equivalent result, so a dose calculated from a female average will often underperform.

Brow shape differs too. A masculine brow sits lower and flatter, whereas the arched, slightly elevated brow many female patients prefer requires different placement laterally. Consequently, treating a man with a plan designed to lift and arch the brow risks a feminised appearance he did not ask for. We plan for the shape you actually want, and we ask rather than assume.

If This Is Your First Treatment

Our advice is consistent: start small and start with the glabella. Frown lines are the area where the effect is most reliable, the anatomy most forgiving, and the risk of unwanted brow change lowest. You then see how your face responds to treatment before committing to more.

Come back at two weeks and we will assess together. If you want the forehead treated as well, that can be added then with the glabella already softened, which is the safer sequence. Adding is straightforward; waiting out an over-treated forehead takes three to four months, and nothing shortens it. Any clinic that presses a first-time patient into three areas at full dose in one sitting is selling rather than advising.

On “Baby Botox” and Micro-Dosing

Micro-dosing means smaller amounts spread across more points, aiming to soften movement rather than stop it. It suits younger patients, those with fine lines, and anyone whose work depends on facial expressiveness.

Two honest caveats, though. Results are subtler, which is the point, but some patients expecting a dramatic change are disappointed. Duration also tends to be shorter, often closer to two to three months, so the annual cost may not be lower despite the smaller dose per session. It is a legitimate approach rather than a magic one, and we will tell you which category you fall into.

What to Expect

Treatment itself takes around 10 minutes for the upper face, using a very fine needle, and anaesthetic is not usually required. Onset is three to seven days with the full effect settled by around two weeks, and duration is typically three to four months.

Immediately afterwards, stay upright for four hours and avoid rubbing or massaging the area for 24 hours. Skip the gym, sauna, steam room and facials for a day. Small bumps and mild redness at the injection points settle within an hour or two, and minor bruising is possible. Risks Specific to the Upper Face

Two are worth distinguishing, because patients often confuse them. Brow heaviness results from over-treating the forehead relative to the depressors, and it resolves as the effect wears off. Eyelid ptosis, a drooping upper lid, is uncommon and occurs when product affects the muscle that lifts the eyelid; it also resolves, though it can take weeks and is more visible.

Asymmetry is usually correctable at the two-week review, which is precisely why that appointment exists. Bruising and a mild headache in the first day or two are common and settle without intervention. We review every patient at two weeks rather than assuming the first dose was perfect.

Who Should Not Have It

Treatment is avoided in pregnancy and breastfeeding, with infection at the injection site, with known allergy to any component including albumin, and in neuromuscular conditions such as myasthenia gravis. Can I have just my forehead treated?

We would generally advise against it. The forehead muscle is the only one that lifts your brows, so weakening it while leaving the depressors untouched makes the brow sit lower and can create a heavy, hooded feeling. Forehead treatment is planned alongside the frown lines for that reason.

Can I have just my frown lines treated?

Yes, and it is often the ideal first treatment. Treating the glabella alone is straightforward, reliable, and frequently gives a slight brow lift as well. It is our usual recommendation for a first-time patient.

Will Botox lift my brows?

Both resolve as the effect wears off, though the eyelid takes longer and is more noticeable.

Do men need more units?
