Smile lines treatment in Dubai is one of the most common things patients ask for, and one where the obvious answer is usually the wrong one. Most people point at the crease running from nose to mouth and ask us to fill it. Frequently the better result comes from treating somewhere else entirely, because that crease is often a shadow cast by volume loss higher in the face rather than a problem in the line itself. First, Some Smile Lines Are Meant to Be There

Everyone has nasolabial folds when they smile. They are the boundary between the cheek and the upper lip, and they exist in babies. The aim of treatment is never to remove them, because a face with no nasolabial definition at all looks distinctly wrong — flat, inflated and difficult to place.

What is treatable is a fold that has become deep at rest, or one that has developed a shadow or a hollow above it. That distinction — deep when you smile versus deep when your face is relaxed — is the first thing we assess, and it changes the plan entirely.

Three Different Lines, Often Confused

Patients frequently describe all three as smile lines, and they respond to quite different treatments. Getting the label right in the consultation matters more than it sounds.

Why Filling the Fold Is Often the Wrong Move

This is the argument worth understanding before you spend anything. As the midface loses volume with age, the cheek descends slightly and its weight comes to rest above the nasolabial fold. The fold deepens not because it has lost anything, but because there is now heavier tissue sitting on top of it.

Inject filler directly into that crease and you are adding volume beneath tissue that is already overloaded. The result can look heavy, pillowy and oddly full around the upper lip, and in some patients the fold appears no shallower for it. Meanwhile, restoring support in the cheek lifts the tissue off the fold, and the crease softens as a consequence — often more convincingly, and in an area that is technically safer to treat.

So when we suggest treating your cheeks rather than the line you pointed at, that is the reasoning. Some patients need a small amount in the fold as well, and we will do that where it is warranted. But midface first, fold second, and often fold not at all.

That last row is worth stating clearly, because patients sometimes ask for it. The muscles that create the nasolabial fold are the ones that lift your upper lip when you smile. Weakening them produces an asymmetric or restricted smile, so botulinum toxin is not a treatment for smile lines in the way it is for frown lines. We would decline.

Why This Area Needs Care

The nasolabial region carries a significant artery and its branches, and it is one of the recognised higher-risk sites for vascular complications in filler treatment — including tissue loss affecting the side of the nose. That is a further reason we favour treating the midface where possible and use small volumes when treating the fold itself.

What Deepens Smile Lines

Several things beyond age, and some are worth acting on regardless of treatment. Sun exposure degrades the collagen that supports the skin around the fold, and in this climate that is the single largest modifiable factor. Significant weight loss deepens folds by removing midface volume — and if your weight is still changing, we would generally wait before treating. Smoking accelerates the same collagen loss. Sleeping consistently on one side deepens the fold on that side, which is why asymmetry is so common. And genetics determine a good deal of it, including in people far too young to blame ageing.

What to Expect

Treatment takes around 20 to 30 minutes with topical anaesthetic. Expect swelling for a few days and possible bruising for up to two weeks, particularly with direct fold treatment. Avoid pressing or massaging the area, skip the gym, sauna and steam room for 48 hours, and avoid dental work for around two weeks where possible.

Judge the result at two weeks, which is when we review. Filler here typically lasts nine to eighteen months, longer in the midface than in the fold itself, since the mouth area moves constantly.

Realistic Expectations

Softened, not erased. A deep, long-standing fold in an older patient will improve rather than disappear, and anyone promising otherwise is either overselling or planning to overfill you. Deep static folds also respond partially at best, since the crease is etched into the skin as well as shadowed by tissue above it.

We would rather set that out now than have you assess the result against an expectation nobody could meet.

Who Should Not Have Filler Here

Treatment is avoided in pregnancy and breastfeeding, with active infection or inflammation in the area including cold sores, and with known allergy to any component including lidocaine.

These require discussion first: previous filler in the region including anything of unknown type, autoimmune and inflammatory conditions, immunosuppression, anticoagulant and antiplatelet therapy, a keloid tendency, previous adverse reaction to filler, and recent or planned dental work or vaccination. Why do you want to treat my cheeks when the problem is my smile lines?

Because the fold is often deepened by cheek volume descending and resting above it, rather than by anything lost in the line itself. Restoring midface support lifts tissue off the fold, which softens it — usually more convincingly than injecting the crease, and in a technically safer area.

Can smile lines be removed completely?

No, and they should not be. Everyone has nasolabial definition when they smile, and a face without it looks flat and inflated. The realistic aim is softening a fold that has become deep at rest.

Can Botox treat smile lines?

Not appropriately. The muscles involved lift your upper lip when you smile, so weakening them restricts or distorts your smile. We would decline. Botulinum toxin is for lines caused by muscles you can afford to relax, and these are not among them.

Is filler in this area risky?

It is one of the higher-risk filler regions, because a significant artery runs nearby, with tissue loss affecting the side of the nose among the recognised complications. That is why we prefer midface treatment where possible, use small volumes in the fold, and require hyaluronidase on site with an emergency protocol.

What is the difference between smile lines and marionette lines?

Smile lines run from the nose to the corner of the mouth. Marionette lines run from the corner of the mouth downward toward the jaw. They respond to different treatments, so the distinction matters when planning.

How long will it last?

Typically nine to eighteen months, longer in the midface than in the fold itself, since the area around the mouth moves constantly.

Why is one side deeper than the other?

Very commonly because you sleep on that side, and sometimes from asymmetric sun exposure or natural facial asymmetry, which almost everyone has. We treat each side according to what it needs rather than injecting equal amounts.

I have lost weight recently. Should I treat my smile lines now?

If your weight is still changing, we would usually wait. Weight loss removes midface volume and deepens folds, so treating now means paying to replace volume that continued loss will undermine.

Will it look obvious?

Not if the midface is treated appropriately and the fold conservatively. Obvious results in this region come from overfilling the crease directly, which produces a heavy, pillowy look around the upper lip.

What about the fine lines around my mouth?

Those are perioral lines and they are a skin quality problem rather than a volume one. Polynucleotides or microneedling suit them better than filler.

How soon before an event should I book?

At least two to three weeks, since bruising in this area can take up to two weeks to clear and the result is best judged at two weeks.
