The Question to Ask Before Anyone Injects You
We are putting this first rather than at the bottom, because it matters more than anything else on this page.
Ask whether the clinic stocks hyaluronidase on site. Hyaluronidase is the enzyme that dissolves hyaluronic acid filler. It is the emergency treatment if filler blocks a blood vessel, and in that situation it must be given within hours, not days. A clinic that injects HA filler without holding hyaluronidase, in date, with staff trained to use it, is not equipped to manage the one complication that can cause permanent harm.
Ask also whether there is a written emergency protocol for vascular occlusion, what training the injector has in complication management, and how to reach someone after hours. These are entirely reasonable questions. Any competent clinic will answer them readily, and hesitation tells you what you need to know. What Filler Is
Dermal fillers used in the face are almost always hyaluronic acid gels — a substance that occurs naturally in skin, manufactured as a crosslinked gel that holds its shape and binds water. It is placed beneath the skin to restore volume, improve definition or support tissue that has descended.
Two properties matter. First, it is temporary, lasting roughly six to eighteen months depending on the product, the area and your metabolism. Second, and importantly, hyaluronic acid filler is reversible — it can be dissolved with hyaluronidase if the result is unsatisfactory or if there is a complication. That reversibility is precisely why we use HA filler in the face and would counsel against permanent or semi-permanent alternatives, which cannot be undone if something goes wrong.

Note the tear trough row. It is one of the most requested areas and one of the least forgiving, prone to prolonged swelling, a bluish discoloration where product sits too superficially, and fluid retention that can persist for months. We treat it cautiously, and in a meaningful proportion of patients we advise against it, because their hollowing is better addressed elsewhere in the face or not at all.
Serious Risks You Should Understand
Most filler treatments pass without incident. Nevertheless, you are entitled to know the serious risks before consenting, not after.
Vascular occlusion. If filler enters or compresses a blood vessel, the tissue supplied by that vessel loses its blood supply. On the skin this causes pain, blanching, mottling and, untreated, necrosis and scarring. If it affects vessels connected to the eye’s circulation, it can cause visual loss, which may be permanent. This is rare but documented, and it is the reason immediate hyaluronidase and a trained injector matter so much. Highest-risk zones are the nose, glabella between the brows, and the tear trough.
What to do if something is wrong. Contact the clinic immediately — the same day, not at your next appointment — if you develop severe or increasing pain, blanching or white patches, dusky or mottled skin, any visual change, or pain out of proportion to what you were told to expect. Early treatment changes outcomes substantially. Do not wait to see whether it settles.
Other Complications
- Swelling and bruising — expected rather than complications; typically a few days to two weeks depending on area.
- Lumps and irregularity — often settle or are correctable; sometimes need dissolving.
- Migration — product moving from where it was placed, seen most often in lips.
- Tyndall effect — a bluish-grey tint where filler sits too superficially, particularly under the eyes.
- Delayed nodules — firm lumps appearing weeks to months later, sometimes triggered by illness, dental work or vaccination. Treatable, but they need assessing rather than ignoring.
- Infection — uncommon with sterile technique, but possible.
- Overfilling — not a complication in the medical sense, but the most common bad outcome by a wide margin.
Overfilling Is the Real Epidemic
The faces you see and instinctively recognise as “done” are rarely the result of one dramatic error. They are the accumulation of reasonable-seeming additions over years, each one small, none of them refused. Filler does not vanish neatly on schedule either — some persists longer than expected, so topping up on a fixed calendar quietly adds volume that was never fully replaced.
Guarding against this is a clinical discipline rather than a marketing line. In practice it means treating the face as a whole rather than the area a patient points to, using less than the maximum, spacing treatments so results can be judged, and declining. Patients occasionally leave our consultations with no treatment booked. That is a good outcome.
Filler Is Not the Answer to Everything
Three misdirections are worth naming. Filler does not treat lines caused by muscle movement — those respond to botulinum toxin. It does not improve skin quality, texture or pigmentation, which need resurfacing, needling or medical treatment. And it does not lift significantly descended tissue; adding volume to a heavy face makes it heavier, not lifted.
Where genuine laxity is the concern, tightening treatments or a surgical opinion are the honest answers. We would rather refer you than fill a face that needs lifting.
The Appointment
- Assessment — what has actually changed, your medical history, medications, previous filler including what and when, and your goals.
- Consent — product, volume, expected result, duration, and the risks above including vascular occlusion.
- Preparation — skin cleansed and disinfected thoroughly; topical anaesthetic where appropriate.
- Injection — needle or cannula depending on area, with careful technique to reduce vascular risk.
- Review — at two weeks, once swelling has settled and the result can genuinely be judged.
Please tell us about all previous filler, including anything placed elsewhere and anything you are unsure about. Old product changes both the plan and the risk, and “I think it was a few years ago somewhere in Europe” is more useful to us than nothing.
Aftercare
Expect swelling and possible bruising, usually settling within a few days to two weeks depending on area — lips swell most. Avoid the gym, sauna, steam room and swimming for 48 hours, and avoid pressing or massaging the treated area unless we specifically instruct you to. Sleep slightly elevated for the first night or two if swelling bothers you. Avoid dental treatment for around two weeks where possible, since dental procedures are associated with delayed inflammatory reactions. Contact us the same day if anything from the warning list above occurs.
Who Should Not Have Filler
Treatment is avoided during pregnancy and breastfeeding, in the presence of active infection or inflammation at the intended site, including active acne or cold sores, and where there is known allergy to any component including lidocaine in products containing it.
These require discussion beforehand: autoimmune and inflammatory conditions, immunosuppression, anticoagulant and antiplatelet therapy, a keloid tendency, previous adverse reaction to filler, recent or planned vaccination or dental work, and any permanent filler placed previously. A complete history including supplements is taken at assessment.

One caution that matters more here than in most categories: be wary of unusually cheap filler. Product quality and provenance vary, unregistered product circulates, and this is the one treatment where a bad outcome can be permanent.
