The under-eye area is unforgiving, the majority of people who ask for filler here are not good candidates for it, and the wrong patient treated well still ends up unhappy. Six Different Problems, One Appearance
“Dark circles” and “tired eyes” describe an appearance, not a cause. Six quite different things produce a similar look, and only one of them responds to filler.

Most patients have a combination, frequently with hollowing as the smallest component. Consequently, treating everyone who asks with filler produces a lot of disappointed people, which is precisely what has happened across this market.
Malar Oedema: The Complication Nobody Warns You About
This deserves its own section, because it is common, it is distressing, and it is rarely mentioned before treatment.
The under-eye area drains lymph poorly at the best of times. Hyaluronic acid attracts water. Put filler into an area with sluggish drainage and you can produce persistent puffiness — a soft, swollen pad below the eye that looks worse than the hollow you were trying to correct. It is not ordinary post-treatment swelling that settles in a week. It can persist for many months, and it often requires dissolving to resolve.
Certain patients are considerably more prone to it: anyone who already wakes with puffy eyes, anyone with existing fluid retention or festoons, anyone with a history of allergic or sinus problems, and older patients with lax lower-lid skin. If that describes you, we will very likely advise against filler here, and we would rather have that conversation than treat you and manage the consequences.
The Other Ways It Goes Wrong
- Tyndall effect — a bluish or grey cast where product sits too superficially beneath very thin skin. It looks like a bruise that never fades and usually needs dissolving.
- Visible lumps — the skin here is the thinnest on the face, so irregularities that would be invisible in a cheek are obvious under an eye.
- Product lasting years — filler in this area is metabolised slowly and can persist far longer than the twelve to eighteen months quoted, sometimes for several years. That means a poor result is a long-term problem, and it also means top-ups can pile onto product that never went away.
- Worsening the appearance of bags — adding volume beneath a fat prolapse tends to accentuate the bulge above it.
- Vascular occlusion — rare but serious here, with visual loss a recognised risk. The details are on our face filler in Dubai page and apply fully.
What We Often Recommend Instead
Saying no to filler is not the same as having nothing to offer. Depending on what your assessment shows, the better route is frequently one of these.
- Treat the midface instead. A great deal of under-eye hollowing is really loss of cheek support. Restoring volume slightly lower and more laterally often improves the under-eye appearance with none of the periorbital risk.
- Improve the skin rather than the volume. Where thin, crepey skin is the issue, polynucleotides are usually better suited than filler for this area.
- Treat the pigmentation. Where the darkness is pigment, filler achieves nothing and a medical pigmentation plan does.
- Surgical opinion. Where fat prolapse is the dominant feature, lower-lid surgery is the honest answer and filler is not.
- Address the simple things. Sleep, salt intake, allergy control and sinus problems genuinely affect this area, and they cost nothing to address.
- Do nothing. Sometimes the honest recommendation, particularly in patients whose anatomy makes a good outcome unlikely.
If You Are a Suitable Candidate
Where there is genuine hollowing, good lower-lid tone, no marked fluid retention and realistic expectations, this treatment can be excellent. In those patients it is one of the more rewarding things we do.
The technique matters. We use a soft product formulated for the periorbital area rather than a standard midface filler, place it deep against the bone rather than superficially, and use very small volumes — often a fraction of a syringe per side. . We under-correct deliberately and review at four weeks, because in this area it is far better to add a little later than to remove product from beneath thin skin.
What to Expect Afterwards
Bruising is common here, since the area is vascular and the skin thin, and it can take one to two weeks to clear. Swelling in the first few days is normal. Avoid pressing or rubbing the area, sleep slightly elevated for the first few nights, keep salt intake moderate for the first week, and skip the gym, sauna and steam room for 48 hours.
Do not judge the result before four weeks. And contact us the same day if you develop severe or increasing pain, blanching or dusky discoloration, or any change in vision.
Dissolving
We see a steady flow of patients who have had under-eye filler elsewhere and want it removed — for puffiness, for a bluish cast, for lumps, or simply because it never looked right. Hyaluronic acid filler can be dissolved with hyaluronidase, and in this area dissolving is often the correct treatment rather than a failure.
Expect a process rather than an instant fix. The area may look hollow or uneven for several weeks afterwards, more than one session is sometimes needed, and old product can be surprisingly persistent. If you are unhappy with work done elsewhere, come and be assessed. We will tell you honestly whether dissolving, waiting or leaving it alone serves you best, and we are happy to dissolve filler we did not place.
Who Should Not Have It
Beyond the general filler contraindications — pregnancy and breastfeeding, active infection or inflammation in the area, known allergy to any component — we would specifically advise against under-eye filler where there is prominent fat prolapse, established fluid retention or festoons, marked lower-lid laxity, or a history of malar oedema after previous treatment.
Autoimmune and inflammatory conditions, immunosuppression, anticoagulant therapy, a keloid tendency, previous adverse reaction to filler and previously placed permanent filler all require discussion. The full list is on our face filler page.

Volumes used here are small, so product cost is modest. What you are paying for is assessment and judgement — specifically, someone willing to tell you that you are not a candidate.
That is worth stating plainly, because cheap under-eye filler is a false economy in a way that cheap cheek filler is not. Will under eye filler get rid of my dark circles?
Only if the darkness comes from a genuine hollow casting a shadow. Where it comes from pigmentation, thin skin showing underlying vessels, or fat prolapse, filler will not help and may make things worse. Establishing the cause is the whole point of the assessment.
Can filler fix under-eye bags?
No. Bags are fat prolapsing forward, and adding volume beneath them tends to accentuate the bulge. Where bags are the dominant feature, a surgical opinion is the honest recommendation.
What is malar oedema?
