Read This Before Considering Nose Filler Anywhere
The nose and the area between the brows are the two sites most often implicated in filler-related blindness. The reason is anatomical: nasal blood vessels connect with vessels supplying the eye, so filler that enters a nasal artery can travel backwards into the retinal circulation. The resulting visual loss can be permanent. Skin loss on the nasal tip or bridge is also well described.
These events are rare. They are not, however, theoretical, and they are the reason this treatment demands a specific standard from whoever performs it. Before anyone injects your nose, establish four things:
- Does the injector have specific training in nasal filler? General filler experience is not the same thing. This area has its own anatomy and its own techniques.
- Is hyaluronidase held on site, in date, in adequate quantity? It is the emergency treatment and it must be given within hours.
- Is there a written protocol for vascular occlusion, including suspected visual compromise? Ask to see it.
- How do you reach someone urgently, including after hours? You should leave with that in writing.
If a clinic cannot answer all four, do not have your nose injected there. And if a clinic declines to treat noses altogether, that is a legitimate and often sensible clinical position rather than a limitation.

What It Cannot Do — and This Is the Important Part
Filler adds volume. It cannot remove any. That single sentence resolves most of the disappointment we see, because a large proportion of patients asking for nose filler actually want a smaller nose, and this treatment cannot make a nose smaller.
- It will not reduce the size of your nose. It will make a straighter, more defined nose — not a smaller one.
- It will not narrow a wide nasal base or wide nostrils. That is skeletal and cartilaginous structure.
- It will not meaningfully refine a bulbous tip.
- It will not correct a significantly deviated nose. Camouflage of minor deviation is possible; genuine correction is surgical.
- It will not improve your breathing. If anything, adding volume in the wrong place can impair it.
Consequently, if what you want is a smaller or narrower nose, a surgical rhinoplasty opinion is the honest recommendation and we will tell you so. That conversation costs you nothing and saves you from paying for the wrong treatment.
If You Have Had a Surgical Rhinoplasty
This needs stating separately. Previous nasal surgery alters the anatomy and the blood supply and leaves scar tissue, all of which materially increase the risk of a vascular complication and make product behaviour less predictable.
Therefore we approach post-surgical noses with extreme caution, and in many cases we decline. If you have had surgery, please tell us what was done and when, and bring any operative details you have. Where refinement after rhinoplasty is what you want, a return to your surgeon is usually the safer route than filler placed into altered tissue.
How the Treatment Is Done
- Assessment — the shape of your nose, what specifically bothers you, what is realistically achievable, previous surgery or filler, and a full medical history.
- Consent — a full discussion of what can and cannot be achieved, and of the risks above including visual loss. We want you to understand these before, not after.
- Preparation — thorough skin disinfection and topical anaesthetic.
- Injection — very small volumes, placed deep against the bony and cartilaginous framework rather than superficially, injected slowly, with continuous attention to how the skin looks. .
- Observation — you remain in clinic afterwards so we can check the skin before you leave.
- Review — at two weeks, once any swelling has settled.
We use small amounts. Total volumes in the nose are typically a fraction of what is used in cheeks, and treating conservatively is a safety measure as much as an aesthetic one.
Warning Signs: Act the Same Day
Contact us immediately — the same day, by phone, not by waiting for your review — if you develop any of the following after nose filler:
- Any change in vision: blurring, dimming, double vision, loss of part of your visual field, or pain in or behind the eye.
- Severe or escalating pain, particularly pain out of proportion to what you were told to expect.
- Blanching, white patches, or a dusky, mottled or bluish appearance of the skin on the nose, bridge or between the brows.
- Spreading discoloration or increasing coldness of the skin.
If you cannot reach us and you have visual symptoms, attend a hospital emergency department without delay. Time matters enormously in these situations, and no one will think you overreacted.
Normal Aftercare
Mild swelling and occasional bruising are usual and settle within a few days to two weeks. Avoid pressing, massaging or manipulating the nose, and take care with glasses or sunglasses that rest on the bridge for the first week or two. Skip the gym, sauna, steam room and swimming for 48 hours, and sleep on your back if you can for the first couple of nights. Avoid dental work for around two weeks where possible.
Duration
Nose filler often lasts longer than filler elsewhere — commonly nine to eighteen months, sometimes more — because the nose moves relatively little. That is generally welcome, but it also means product may still be present when you consider a top-up, so we assess what remains rather than assuming it has gone.
Who Should Not Have It
Treatment is avoided in pregnancy and breastfeeding, with any active infection or inflammation of the nose or surrounding skin, with known allergy to any component including lidocaine, and where permanent or semi-permanent filler has previously been placed in the nose.
These require careful discussion and may mean we decline: previous surgical rhinoplasty, previous nasal filler of unknown type, autoimmune or inflammatory conditions, immunosuppression, anticoagulant and antiplatelet therapy, a keloid tendency, and any previous adverse reaction to filler.
