How It Works

Sweat glands are activated by a chemical messenger released from nerve endings. Botulinum toxin blocks that messenger locally, so the glands in the treated area stop being told to produce sweat. The effect is confined to where the product is placed, and sweating elsewhere on the body continues normally.

Two points patients often ask about. First, the glands are not damaged or removed — the signal is simply interrupted, and normal function returns as the effect wears off. Second, this treatment does not cause the compensatory sweating elsewhere that can follow surgical approaches to hyperhidrosis, because nothing is cut or permanently altered.

First: Is It Primary Hyperhidrosis?

Excessive sweating has two broad categories, and treating the wrong one wastes your money and can delay a diagnosis that matters.

Primary focal hyperhidrosis affects specific sites — typically underarms, palms, soles or scalp — usually starts in childhood or adolescence, often runs in families, is generally symmetrical, and does not occur during sleep. This is the type that responds well to treatment.

Secondary hyperhidrosis is generalised sweating caused by something else: thyroid overactivity, diabetes and blood sugar fluctuation, certain medications including some antidepressants, infection, menopause, anxiety disorders and, less commonly, other systemic conditions. Sweating that is generalised rather than focal, that began suddenly in adulthood, that wakes you at night, or that comes with weight loss, fever or other symptoms needs investigating rather than injecting.

Consequently we take a proper history and arrange blood tests where the pattern suggests it. If your sweating turns out to be secondary, treating the underlying cause is both more effective and considerably cheaper.

Try the Simpler Options First

We would rather you spent less if less will work. For underarm sweating, prescription-strength aluminium chloride antiperspirants are the established first step and help a substantial proportion of patients. Applied correctly — to completely dry skin at night, not in the morning after a shower — they are considerably more effective than most people find them, because most people apply them wrongly.

Topical glycopyrronium preparations, iontophoresis for palms and soles, and oral anticholinergic medication are other options, each with their own trade-offs. Botulinum toxin is generally considered where topical measures have failed or are not tolerated, which is a large group of patients but not everybody.

Insurance: Worth Checking Before You Pay

This is the part most clinics do not mention. Because hyperhidrosis is a recognised medical condition rather than a cosmetic complaint, many insurance policies in the UAE will consider covering treatment, typically subject to pre-approval and often requiring documentation that topical treatments have been tried and failed first.

Therefore we suggest you contact your insurer before booking, ask specifically about coverage for hyperhidrosis treatment, and ask what documentation they require. Even where treatment itself is not covered, the consultation and any investigation for secondary causes often are.

The Appointment

  1. Assessment — history, pattern and distribution of sweating, impact on daily life, medications, and screening for secondary causes with blood tests where indicated.
  2. Mapping — the affected area is identified and marked out, so injections cover the region that actually sweats.
  3. Comfort measures — topical anaesthetic, cooling or other measures according to the area treated.
  4. Injections — a grid of small, shallow injections across the marked area, roughly one to two centimetres apart.
  5. Aftercare — instructions given, and a review arranged.

Underarm treatment takes about 20 to 30 minutes including preparation, and dosing is set per underarm according to the area mapped at assessment.

Preparing for Underarm Treatment

Two practical points make the appointment easier. Shave the area 24 hours beforehand rather than on the day, so the skin is not freshly irritated. Also stop using antiperspirant for around 24 hours before, since residue interferes with the mapping.

Otherwise no preparation is needed, and you can return to work immediately afterwards.

Onset, Duration and What to Expect

Reduction usually begins within two to seven days and is complete by around two weeks. Duration for underarms is typically four to seven months, which is longer than most facial areas, and some patients get longer still. Palms tend to last a somewhat shorter period.

Most patients describe the difference as substantial rather than partial — not merely less sweating but the practical freedom to wear what they like and stop planning around it. That said, results vary, and a small proportion of patients respond less well than expected.

Aftercare

Avoid the gym, sauna, steam room and hot baths for 24 hours. Do not massage the treated area for the rest of the day, and leave antiperspirant off until the following day. Mild tenderness, small bumps at the injection points and occasional minor bruising are normal and settle quickly.

You can shower normally, work normally and go about your day. There is no meaningful downtime with underarm treatment.

Risks

Underarm treatment is well tolerated, with bruising, tenderness and injection-point bumps the usual findings. Some patients notice a slight increase in sweating elsewhere, which is generally minor and quite different from the compensatory sweating associated with surgical approaches.

Palm treatment carries one specific risk worth understanding: temporary weakness of grip or fine hand movements, because the small muscles of the hand sit close to the sweat glands being treated. It is usually mild and resolves within weeks, but if your work depends on fine hand function — a musician, surgeon or dentist, for instance — tell us, because it changes how and whether we treat. Scalp and forehead treatment requires care to avoid affecting brow position.

Who Should Not Have It

Treatment is avoided in pregnancy and breastfeeding, with infection at the injection site, with known allergy to any component including albumin, and in neuromuscular conditions such as myasthenia gravis. Tell us about aminoglycoside antibiotics and other medicines affecting neuromuscular transmission, anticoagulant therapy, and any previous reaction to botulinum toxin.
