Why the Two Work Together

The reasoning is mechanical rather than mystical. PRP contains large molecules that cannot cross intact skin, so applying it to an unbroken surface accomplishes very little. Microneedling solves that by creating thousands of temporary micro-channels, and PRP delivered at that moment reaches the dermis rather than sitting on top of it.

The Two Delivery Routes

Clinics do this in two ways, and the distinction is worth understanding when comparing quotes.

Topical application onto freshly needled skin. The PRP is spread across the treated area during and immediately after the needling passes, entering through the open channels. This is the more common approach and the one usually meant by the media nickname “vampire facial”.

Injection followed by needling. PRP is injected into the skin as micro-droplets and the needling is performed in the same session. This places the material more deliberately at depth, and it is sometimes preferred for established scarring.

Acne Scarring: The Flagship Indication

This combination exists mainly for scars, so let us be specific about which ones. Rolling and boxcar scars respond best, since their broad, shallow geometry is what needling addresses well. Improvement builds across a course, typically three to six sessions about four weeks apart, and collagen continues remodelling for months afterwards.

Deep ice-pick scars, by contrast, respond poorly to needling with or without PRP. Their narrow vertical shape is not what this treatment reaches, and adding PRP does not change that. One prerequisite matters more than the choice of protocol: active acne must be medically controlled first. Treating scars while new inflammatory lesions are still forming simply produces more scarring to treat later. If that describes you, a dermatology consultation is the right first appointment.

  1. Assessment — scar type, skin type, medical history, medications and blood-related conditions reviewed, and baseline photographs taken.
  2. Blood draw — a small sample from your arm, as for a routine blood test.
  3. Numbing — topical anaesthetic applied for around 30 minutes while the sample is processed.
  4. Preparation — the sample is centrifuged and the platelet-rich fraction prepared under sterile conditions.
  5. Microneedling — performed systematically, with depth varied by facial zone and by scar depth.
  6. PRP delivery — applied or injected while the channels remain open.
  7. Finish — calming layer, SPF and written aftercare.

Downtime Stacks — Plan for It

Combining treatments combines their recovery, which patients regularly underestimate. Expect redness resembling moderate sunburn for 24 to 48 hours from the needling, with tightness and mild swelling. The PRP component adds a little more swelling, usually most noticeable the morning after, and bruising is possible where PRP is injected rather than applied. Light flaking often follows over the next few days.

Aftercare is stricter than for either treatment alone, because the skin barrier is genuinely open. Leave makeup off for 24 hours. Avoid the gym, sauna, steam room and swimming for 48 hours, since sweat and chlorine on fresh channels risk irritation and infection. Pause retinoids, acids and vitamin C for around five days. Avoid anti-inflammatory painkillers such as ibuprofen for a few days either side where you safely can, since they act on the platelet activity the PRP relies on — paracetamol is generally fine. Sun protection daily, without exception.

Sessions, Timeline and Event Planning

Three to six sessions spaced about four weeks apart is the usual course for scarring, with the number depending on severity. Skin often looks brighter within a fortnight, but the meaningful scar improvement builds over three to six months as collagen remodels. Judge the result at four to six months, not four weeks.

For weddings and events, finish a course at least two months beforehand, and never book a first session within a month of the date. This is not a treatment to try for the first time close to an occasion.

Suitability

Because this combination includes PRP, blood-related conditions matter alongside the usual needling considerations. Treatment is avoided where there is a platelet disorder or low platelet count, active infection or inflammatory acne in the treatment area, and during pregnancy and breastfeeding. Anaemia should be corrected first, since preparation quality depends on it.

These require discussion beforehand: anticoagulant and antiplatelet therapy, blood-borne infection, active or recent malignancy, autoimmune disease, immunosuppression, poorly controlled diabetes, keloid tendency, a history of cold sores in the treatment area, and recent systemic isotretinoin. For general texture and pore refinement without scarring, plain microneedling delivers most of the benefit, and the money is often better spent on more sessions.
