What Is a Skin Lesion Excision?

Excision means the lesion is cut out completely, together with a small margin of surrounding skin where appropriate, and the wound is closed with sutures. It differs from shave removal, which takes a lesion down to skin level, and from cautery or laser destruction, which removes tissue without preserving it for analysis. Excision is the preferred approach when the whole lesion needs to be removed, when it extends below the skin surface, or when laboratory examination of the tissue is important.

When You Should Have a Lesion Checked

Book an assessment promptly if a mole or spot is changing in size, shape or colour, has an irregular or blurred border, contains more than one colour, itches, bleeds, crusts or fails to heal, or simply looks different from your other moles. New pigmented lesions appearing in adulthood also warrant a look. A changing lesion is a reason to be examined, not a reason to panic — most turn out to be benign, and the ones that are not are far more straightforward to manage early.

Please do not attempt to remove any lesion yourself, and avoid home removal kits, thread-tying or cauterising pens. Beyond infection and unnecessary scarring, self-removal destroys the tissue that a laboratory would need to examine — which means a potentially important diagnosis is lost.

The Procedure, Step by Step

1. Assessment — examination of the lesion, your medical history, medications including blood thinners, allergies, and your scarring and keloid history.
2. Consent — an explanation of the method, the expected scar, the risks and the alternatives, including leaving the lesion alone.
3. Local anaesthetic — the area is numbed by injection. You stay awake throughout and should feel pressure rather than pain.
4. Excision — the lesion is removed with an appropriate margin, and the wound is closed with sutures. Most single excisions take around 20 to 45 minutes in total.
5. Histopathology — where clinically indicated, the tissue is sent to a laboratory for examination. We tell you the expected turnaround when the sample is sent, and results are discussed with you directly.
6. Follow-up — wound review and suture removal, usually within one to two weeks depending on the site.

Recovery and Aftercare

You can normally drive yourself home and return to desk work the same day or the next. Keep the wound clean and dry as instructed, avoid swimming, saunas and heavy exercise until sutures are removed, and protect the healing scar from sun exposure for several months, since UV exposure on new scar tissue tends to darken it. Some discomfort, bruising and swelling in the first few days is normal. Contact the clinic if you develop increasing pain, spreading redness, discharge or fever.

Scarring: An Honest Note

Every excision leaves a scar. That is unavoidable — the aim of careful technique and good aftercare is a scar that is fine, flat and inconspicuous, not one that is invisible. The area is numbed with local anaesthetic first. You may feel a brief sting as the anaesthetic goes in, then pressure or movement rather than pain. Mild soreness for a day or two afterwards is normal and usually manageable with simple pain relief.
