What Exosomes Are

Exosomes are extremely small vesicles that cells release naturally, and their role in the body is communication. Each carries a mixture of proteins, lipids and genetic messengers, which it delivers to other cells. In effect, they act as biological signals rather than as building blocks.

The reasoning behind using them on skin follows from that. If exosomes carry signals associated with repair, then applying them to skin that is actively repairing may support the process. That is the theory, and it is a reasonable one. Whether it translates into a measurable clinical advantage in every case, however, is exactly what the research is still working out.

Sources vary considerably, and this matters more than most patients realise. Products may be plant-derived, salmon-derived or derived from human cells , and each category is regulated differently. Consequently, we tell you precisely which product we use and where it comes from, and you are entitled to ask that of any clinic.

Why the Two Are Combined

The pairing is not arbitrary. Exosomes are large molecules that cannot meaningfully cross intact skin, so applying them to an unbroken surface achieves very little. Microneedling changes that, because it creates thousands of temporary micro-channels. Applied immediately afterwards, therefore, the product reaches the skin rather than sitting on top of it.

Timing is central to this. The channels close within hours, which is why the exosome preparation is applied during and directly after the needling pass rather than at a later appointment. In our protocol the product is applied topically to freshly needled skin.

An Honest Account of the Evidence

We want to be careful here, because exosomes are marketed extremely aggressively and the language used is often far ahead of the data. Laboratory studies are encouraging. Early clinical work on skin quality and post-procedure recovery is also reasonably promising. Nevertheless, the published human evidence remains limited, study sizes are small, products differ enormously between suppliers, and long-term data is not yet available.

So here is our position. Microneedling is the component doing the established work, particularly for acne scarring and texture. Exosomes are a plausible and generally well-tolerated addition that may improve comfort, redness and recovery afterwards, and possibly skin quality. They are not, though, a substitute for the needling itself, nor are they a regenerative therapy in any literal sense. Anyone describing exosomes as stem cell treatment, cell renewal or skin regeneration has moved well beyond what the evidence supports.

If that level of caution puts you off the treatment, we would rather it did so now than after you had paid for a course.

What the Combination May Help

Accordingly, some patients find that worthwhile, particularly those with sensitive skin who want an easier recovery, or those who simply prefer the more comprehensive option. Others are better served by putting the same budget toward more microneedling sessions instead. What Happens During Treatment

  1. Assessment — your concerns, medical history, medications, allergies, scarring tendency and previous treatments are reviewed.
  2. Consent — we explain the evidence position honestly, the expected downtime and the realistic benefit, then confirm you want to proceed.
  3. Numbing — topical anaesthetic is applied and left for around 30 minutes.
  4. Preparation — the skin is cleansed and disinfected thoroughly.
  5. Microneedling — the device is passed methodically across the area at a depth chosen for your concern.
  6. Exosome application — the preparation is applied to the freshly needled skin while the micro-channels remain open.
  7. Finish — a calming layer and SPF, plus detailed aftercare instructions.

Allow roughly 75 to 90 minutes including numbing time.

Downtime and Aftercare

Expect redness resembling moderate sunburn for 24 to 48 hours, along with tightness and mild swelling. Light flaking may follow over the next few days. Patients frequently report that redness settles a little faster with the exosome application, although individual experience varies.

Aftercare is strict for a reason, since freshly needled skin is temporarily vulnerable. Leave makeup off for 24 hours. Additionally, avoid the gym, sauna, steam room and swimming for 48 hours, because sweat and chlorine on open micro-channels risk irritation and infection. Pause retinoids, acids and vitamin C for around five days. Finally, sun protection is non-negotiable, as treated skin pigments readily in this climate and neglecting SPF can turn a good result into a pigmentation problem.

Sessions and Timing

A course of three to six sessions spaced about four weeks apart is typical, with the number depending on your concern. Acne scarring generally needs the fuller course. Because collagen continues remodelling afterwards, improvement builds for several months once the course is complete.

For events, finish a course at least three to four weeks beforehand. Never book a first session in the fortnight before something important, given the redness involved.

Who Should Not Have It

Treatment is avoided during pregnancy and breastfeeding, and on skin with an active infection or inflammatory acne flare in the area being treated. Furthermore, a history of keloid scarring, bleeding disorders, blood-thinning medication, immunosuppression, active autoimmune disease and recent oral isotretinoin all need discussing beforehand. Allergies must also be declared, including to any component of the exosome preparation. Please disclose everything at assessment, supplements included.
